The patient was a 68 year old man with an extensive past medical history which included hypertension, dyslipidemia, uncontrolled type II diabetes, alcohol abuse, multiple cerebrovascular events (1994, 1998) with left hemiparesis, vitamin D deficiency, and benign prostate hyperplasia. Left cerebellar and right pontine infarcts dated from 1998. His last admission in May 2011 was for left foot and toe cellulitis, At that time he had uncontrolled diabetes with a HbA1C of 13.7%), adequately controlled hypertension and vitamin D deficiency. He was on statin and aspirin therapy for cerebrovascular stroke prevention. He expired in September of 2011.
