Case 5 History ---- The patient was a 64-year-old woman with a history of Wolfram syndrome (including cognitive delay, optic atrophy, and neurogenic bladder), diabetes, obstructive sleep apnea, recent pulmonary embolism and sepsis secondary to urinary tract infection. She presented to an outside hospital with shortness of breath. In the ED her vital signs were significant for hypoxia (87%) saturation and tachycardia and she was admitted for acute hypoxic hypercapneic respiratory failure secondary to multifocal pneumonia and for acute kidney injury, obstructive sleep apnea, leukocytosis and recurrent pulmonary embolism. She was transitioned to comfort care and passed away. ---- At autopsy the weight of the fixed brain was 970 g. Both optic nerves appeared atrophic and discolored. Coronal sections of the cerebral hemispheres revealed small bilateral lateral geniculate nuclei (arrowheads).
