11A0 Case 11 History
Case 11 History ---- The patient was a 63 year old man status post orthotopic liver transplant in 4/2000 who finally succumbed to complications of liver failure in June of 2000. Neuropathological examination was requested because he was noted incidentally to have an abnormality of the temporal lobe on gross post mortem examination of his brain. He had no known neurologic history or localizing neurologic signs other than his presumed hepatic encephalopathy. Past medical history was remarkable for cryptogenic cirrhosis diagnosed in 1998, nephrolithiasis, cholelithiasis, thrombocytopenia, esophageal varices, a rectal polyp later found to be a tubular adenoma, and facial bone reconstruction secondary to a touch football injury. He was transferred from an outside hospital in early April 2000 with worsening liver dysfunction, septic shock, hepatorenal syndrome, anemia, thrombocytopenia, coagulopathy, and hepatic encephalopathy. He underwent orthotopic liver transplant (April 2000) and temporary portocaval anastomosis but suffered from failure of multiple systems. Support was withdrawn and he passed away 6 weeks later.
