9A0 Case 9 History
Case 9 History ---- At his initial resection in 2011, he was a 52 year old man with a history of a skull fracture at the age of 14 years from a bicycle accident. He subsequently had multiple ear surgeries. In 2024 he was also involved in a motor vehicle accident and, though originally did not seek medical attention, had a complicated medical course with multiple evaluations and treatments including a mastoidectomy for a CSF leak and a cholesteatoma that was partially resected at an outside hospital. He has had recurrent CSF leaks and additional surgeries including a ventriculoperitoneal shunt in 2011. MRI from 9/2011 showed not only a right frontal ventricular catheter but also a large T1 hypointense, T2 hyperintense lesion in the left temporal lobe with mass effect. MRI examination in 2011 was consistent with an epidermoid cyst. Operative procedure in 2011 Left middle fossa craniotomy with pathologic demonstration of an epidermoid cyst. At his second surgery in 2024 he was a 65-year-old man who presented with a 7.8 cm cystic lesion at the 2011 resection site concerning for recurrence. Operative procedure: Left temporal craniotomy for endoscope assisted cyst resection
