Instead, the cortex is replaced by large scattered giant neurons (arrows) admixed with numerous balloon or "grotesque" cells (arrowheads), many of which have nuclear features of neurons. The cytoplasm of these cells is glassy, opaque, and brightly eosinophilic in color. Immunohistochemistry of this case is shown in the next two images. (H&E)
