Case 5 History ---- The patient was a 4 day old liveborn female infant who was born at 32 weeks 1 day gestational age. She was delivered at an OSH and transferred to St. Louis Children’s Hospital for higher level of care. The pregnancy was complicated by emergency cesarean section for terminal bradycardia in the setting of new hydrops fetalis. The neonate had severe biventricular cardiac dysfunction with end organ hypoperfusion and severe fluid overload. She had evidence of complete heart block and severe cardiac dysfunction. She was maintained on mechanical ventilation. The family made the decision to redirect care and the patient was terminally extubated. ---- 5A1,2 The immaturity of this section of cerebral cortex, particularly the difficulty of separating gray cortex from myelin deficient developing white matter only hints at the polymicrogyric cortex. The white matter demonstrates perivascular discolorations (arrows, 5A2), many of which will be subsequently identified as micronodular subcortical neuronal heterotopias.
