This patient was a 5 month old boy born at 29 weeks of gestation with perinatal surgery for a thoracic myelomeningocele as part of Arnold-Chiari malformation. Over his long postsurgical course he developed multiple systemic infections culminating in pneumonia superimposed on chronic pulmonary disease.
