Case 1 History ---- This 79 -year-old man with a history of type II diabetes mellitus, endstage renal disease on hemodialysis, congestive heart failure, hypertension, coronary artery disease, and dementia was admitted for progressive decrease in mental status, lethargy, and inanition for four days, as well as fevers for two days (38.1 oC on presentation to the emergency department). CSF studies found an elevated nucleated cell count with neutrophilic predominance, and he was treated for bacterial meningitis. The patient then fell on hospital day #1, and the follow-up head CT found a multi-septated rim-enhancing 4 cm mass involving the left masticator space with erosions of the left mandible. A later (day #10) head CT found the left masticator abscess had developed into a left subdural empyema through the foramen ovale. He developed PEA arrest and passed away. His brain weighed 1200 grams.
