1B2,3 There is an unusual appearance to the gross specimen. In the most superior part of the specimen there is opacification of the leptomeninges by a subarachnoid infiltrate with preservation and visibility of the cortical vasculature as expected in infection/organization of the subarachnoid space (arrowheads, 1B3). Additionally, there is superimposition of purulent material (arrow, 1B3) external to the arachnoid pia corresponding to the subdural empyema.
