The post fixation specimen shows that there is opacification of the leptomeninges by a subarachnoid infiltrate with preservation and visibility of the superficial cortical vasculature as expected in infection/organization of the subarachnoid space (arrowheads). Additionally, there is superimposition of purulent material external to the arachnoid pia (arrow) with opacification corresponding to the subdural empyema.
