1A0 Case 1 History
Case 1 History ---- The decedent was a 24-day-old male born at 37 weeks of gestation. He developed a mild fever with fussiness, decreased feeding and urine output with irregular breathing patterns. Lumbar puncture (glucose <10 mg/dl, protein elevated at 379 mg/dL and 31 nucleated cells, 90% of which were monocytes) was consistent with bacterial meningitis. Culture showed Group B Streptococcus. Brain MRI demonstrated meningeal enhancement and multifocal infarction in frontal and temporal lobes bilaterally and in the basal ganglia and cerebellum. He experienced further neurologic deterioration, requiring ventilator support, and developed signs of ischemia with Doppler imaging demonstrating a lack of flow through the posterior tibial and dorsalis pedis arteries. Care was redirected and he died.
