Case 2 History ---- The patient was a 21 year old female with a history of seizure disorder and alcohol abuse. She presented with altered mental status and a history of tonic/clonic seizures followed by headache and vomiting 4 days prior to ER presentation. She developed a low grade fever and had a decline in her mental status. Her pupils were reportedly minimally reactive on admission. Fever increased to 40oC and she became hypotensive with clinical signs of herniation with fixed pupils. A second exam confirmed brain death. Her tracheal aspirate grew staph aureus and blood cultures grew gram positive cocci in pairs later differentiated as streptococcus pneumoniae.
