Case 9 History ---- This patient was a 49 year old man with a history of IV heroin abuse, hepatitis C, mitral valve endocarditis and embolic stroke, who
presented to the ER with nausea and vomiting, soon followed by hypotension. The lateral aspect of the prosthetic mitral valve annulus was highly suspicious for vegetations. The patient’s condition worsened, and an MRI demonstrated a large acute left frontal intraparenchymal hemorrhage with intraventricular extension and uncal, transtentorial, and tonsillar herniation. He was transferred to our hospital where on arrival his pupils were fixed and unequal, he had no cough, gag, or corneal reflexes and was pronounced dead.
