Case 1 History
The patient was a 22 year old man with a past medical history significant for infantile Group B Streptococcal meningitis at the age of 13 months with resultant neurologic devastation, hypothyroidism, seizure disorder, blindness, deafness, hydrocephalus, diabetes insipidus, asthma, chronic renal failure, and ventriculo-peritoneal shunt. Over the past several years, he has had frequent hospital admissions for pneumonia, renal failure, myxedema coma, and seizures secondary to hypernatremia. The patient presented with an episode of emesis after his morning meal, increased work of breathing, and suspected aspiration. He was lethargic febrile, tachycardic, and was found to have severe hypotension, poor peripheral perfusion, and signs of decompensated shock, and died.
