Case 3 History
The patient was born at 25 weeks of gestation and survived 3 months. Postnatal ventilatory support was followed by a pulmonary hemorrhage and Klebsiella pneumonia. Seizure activity was noted at one week. He developed bilateral IVHs-Grade 4 on the left and grade 3 on the right. These lesions required placement of a right frontal reservoir for worsening hydrocephalus. One week later CSF culture yielded Klebsiella pneumonia. He was continued on vancomycin and gentamicin for presumed ventriculitis. Serial ventricular taps were performed every 2-3 days. MRI revealed ventriculitis in the lateral, third and forth ventricles, hemorrhagic blood products in the lateral ventricles and aqueductal stenosis with entrapment of the fourth ventricle. Multiple septa were noted preventing adequate drainage of CSF. Care was redirected.
