Case 3 History ---- (AANP Diagnostic Slide Session 1995, Case 6) This was a 34 year old woman with a history of AIDS, dementia, MAI infection, candida esophagitis, refractory anemia and genital herpes. She presented with multiple complaints, including decreased oral intake and inability to take medications, dysphagia, nausea, chronic abdominal pain, confusion and a questionable seizure on the day prior to admission. She was disoriented on admission. Her condition slowly deteriorated and she died.
Gross findings were a 1150 gram brain with a periventricular area of granularity. She had disseminated MAI infection in non-CNS areas and the CNS lesion was thought most compatible with this pathogen group. She had multinucleated HIV+ cells and CMV in other areas of the brain. A final diagnosis was made of cerebral mycobacteriosis, and not further classified.
