Case 3 History ---- Intraluminal Megakaryocyte Occlusion of Small Vessels. The patient was a 3-year-old male with hypoplastic left heart syndrome S/P multiple cardiac surgeries and decompensated heart failure. He developed a hypercoagulable state and developed multiple thrombi in his right ventricle, left internal jugular, left subclavian, and innominate vein requiring thrombectomy. He was placed on ECMO followed by a ventricular assist device. His circuit was replaced multiple times due to frequent accumulation of fibrin and thrombi, despite anticoagulation with heparin, AT3 infusion, as well as antiplatelet agents. Head MRI showed diffuse cerebral volume loss early but later demonstrated multiple areas of infarction. His level of alertness progressively decreased over the final days of his life. The family decided to redirect care.
