Case 2 History ---- This 39 year old man was in normal health until he returned from an outdoor adventure having developed a small ankle bite injury. Within a week he developed a petechial rash, fever (to 104oF), headache, vomiting, and confusion, soon lapsing into coma punctuated with seizures. Serologies were negative for typhoid, paratyphoid, tularemia, salmonella groups B&C, leptospirosis, meningococcemia, brucella and proteus OX19 early on. CT scan demonstrated small ventricles and edema. Antibiotics received included chloramphenicol and penicillin; decadron was also administered. He remained comatose with frequent focal and grand mal seizures. He deteriorated rapidly and expired two weeks after onset of symptoms of apparent vasomotor collapse.
