Case 1 History ---- The patient was a 54 year old man with no known past medical history (except a two pack per day smoker and alcohol abuse) who presented with a 3-4 week history of altered mental status described as personality changes, progressing to gait instability and urinary incontinence. Imaging showed multiple rim-enhancing lesions and ventriculitis. Imaging of the chest showed right upper lobe cavitation. Ultimately, a frontal brain biopsy was performed and pathology was positive for AFB organisms. The patient recovered with multidrug therapy.
