Case 2 History ---- The patient was a 47 year old man with diabetes mellitus, hypertension, hyperlipidemia, and coronary artery disease who presented with an acute myocardial infarction and cardiogenic shock resulting in placement of an intra-aortic balloon pump. Throughout his post-operative course he was noted to have altered mental status with waxing and waning levels of alertness. A repeat head CT revealed development of marked cerebral edema with bilateral transtentorial herniation. Post mortem analysis of CSF revealed Mycobacterium tuberculosis.
