Case 6 History ---- The patient is a 57 year old man with a history of a cerebrovascular accident with residual spastic right hemiparesis. He presented to an outside hospital after being found unresponsive. On imaging he had a right apical lung infiltrate concerning for an infection, thought likely tuberculosis or fungus. MRI showed a T2 hypointense mass in the left frontal lobe with marked surrounding vasogenic edema. Tissue analysis by DNA probe hybridization was positive for mycobacterium tuberculosis complex.
