Case 1 History ---- This patient was a previously healthy 70 year old man with a 2 month history of fever, headache and confusion who was thought to have a subacute meningitis (CSF 320 WBC, 90% lymphocytes) although an organism was never cultured, revealed by PPD, India ink test or CSF cryptococcal antigen. CT scan demonstrated a basal ganglia lesion which was biopsied with CT guidance and demonstrated macrophages and endothelial prominence in the absence of any organisms, consistent with an infarct. Later lumbar puncture resulted in the patient developing a seizure and sudden death.
