Case 4 History ---- The patient was a 54 year-old HIV-negative man with Crohn's disease who had a recent flare treated by prednisone taper. A lumbar puncture showed an infectious picture (329 nucleated cells, 88% of which were neutrophils). He developed hyperglycemia and acidosis. Head CT revealed right frontal lobe hypodensity and maxillary, bilateral ethmoid, and frontal sinus disease. A nasal swab demonstrated septated hyphae and culture showed Rhizopus. He was started on Amphotericin B, but he continued to deteriorate clinically and progressed to brain death.
