Higher magnification of a myelinopathic/axonopathic lesion in which myelin pallor, numerous macrophages and axonal spheroids are noted. This pattern is also reminiscent of perivenous encephalomyelitis (ADEM), however, it is more heterogenous, more axonopathic and lacks a significant lymphocytic component. There is no evidence of PML histopathology or CMV inclusions (LFB-PAS)
