Case 5 History ---- This patient was a 36 year old male diagnosed with HIV/AIDS. At the time of diagnosis, his CD4 count was 8. He began to experience poor concentration and balance, blurred vision, and fatigue. He was admitted for evaluation of two weeks of fever, worsened fatigue, and new onset headache. Neurologic evaluation revealed mild ataxia and brain MRI showed symmetric areas of bright T2 signal without enhancement in the cerebellar white matter bilaterally extending to the middle cerebellar peduncles. CSF PCR was positive for JC virus. He was discharged to a nursing home where his disability progressed and he expired.
