1A2,3 T1-weighted MRI is hypointense in typical PML lesions (arrows) but does not typically enhance with contrast. PML-IRIS T1-weighted lesions maintain hypointensity in non-contrast images but often show enhancement, usually patchy as in this case (arrows, 1A3), with contrast administration.
