5A0 Case 5 History
Case 5 History ---- The decedent was a 54 year old female, with a past medical history of obstructive sleep apnea, COPD, bilateral DVT, alcoholism, and alcoholic encephalopathy. EEG showed moderate generalized slowing with left temporal sharp waves, thought consistent with metabolic encephalopathy. MRI showed a persistent area of mild restricted diffusion and slight decreased FLAIR hyperintensity in the splenium of the corpus callosum which was suggestive of nutritional deficiencies or a metabolic disturbance. She developed increased oxygen demand and CXR showed bilateral patchy opacities concerning for aspiration event. Care was redirected and the patient expired. 5A1-4 This patient showed Morel’s laminar sclerosis, alcoholic cerebellar degeneration and Marchiafava-Bignami disease (but not mamillary body changes) at autopsy.
