14 Kernicterus (Case 14) 1
Case 14 History ---- This is the case of an 8 day old female born after a 32 week gestation in a 23 year old G3P2Ab0 mother who had hepatitis at the time of delivery with a positive Australia antigen. Delivery was vaginal and the baby had spontaneous respiration but grunted and retracted shortly afterward. A diagnosis of Respiratory Distress Syndrome secondary to prematurity was made and she was put on oxygen. At 5 days of age she vomited green bloody material and later developed abdominal rigidity. X-rays were consistent with necrotizing enterocolitis. Next day she had and emergency exploratory laparotomy demonstrating perforations in the stomach and the ileum and resection of gangrenous segments of bowel was performed. She died a few hours after operation. Culture of peritoneal fluid at the time of operation grew Klebsiella. At autopsy she had necrotizing membranous enterocolitis. ---- Involvement of the lateral floor of the IV ventricle.
