Case 2 History ---- The patient was a 63-year old male who had a past medical history of neurosarcoidosis, pulmonary embolism and deep vein thrombosis, multiple sclerosis, rheumatoid arthritis, GERD, histoplasmosis, hip replacement, and squamous cell carcinoma of the skin on the face. He was diagnosed with multiple sclerosis in 1993. He had three strokes in 2002, 2003, and 2005. He was diagnosed with pulmonary sarcoidosis by open lung biopsy at Mayo Clinic in 2003. In April of this year, he started to have episodes of confusion/delirium. Five weeks before his death, he started to have visual hallucinations. He developed fluctuating mental status, hyperglycemia, persistently elevated LFTs, and elevated ammonia. He was also receiving high-dose steroids. The family decided to put him on palliative care due to the poor prognosis.
