Case 4 History ---- The patient was a 42-year-old African-American woman with a past medical history of congestive heart failure, coronary artery disease (status-post stent placement), hypertension, type 2 diabetes mellitus and hypercholesterolemia. She presented complaining of worsening pain in her right great toe for several weeks which was grey in color, swollen and ulcerated, with decreased sensitivity and movement. She was admitted with a clinical diagnosis of acute cellulitis and was treated with intravenous antibiotics. She suddenly developed respiratory distress with agitation followed by cardiac arrest. Attempts at resuscitation were without success.
