There is also an increase in the number of acid phosphatase positive granules in muscle fibers throughout the biopsy. Acid phosphatase positive granules are associated with some hereditary disorders of lysosomal processing as well as myotonic dystrophy. Staining for the three epitopes of dystrophin and the four sarcoglycans as well as dysferlin and adystroglycan show numerous small vacuoles or rounded structures located in many muscle fibers in the biopsy. They are most strongly stained by caveolin. They are not stained well with Laminin. They are mildly outlined by desmin. There are normal levels of staining for all the above antibodies on the surface membrane. There is normal staining for emerin. ---- NM Division Impression : Abnormal dystrophy-related protein consistent with adult onset autophagic vacuolar myopathy.
