Case 5 History ---- The patient is a 20 year-old woman, with progressive proximal weakness since childhood and an elevated creatine kinase (761 IntUnits/L). EMG studies show an irritable myopathy with myotonic discharges as can be seen with inherited myotonic disorders and metabolic myopathies such as acid maltase deficiency, or in myofibrillar myopathies. Operative procedure: Left deltoid muscle biopsy. ---- 5A1-3 Some of the muscle fibers contain empty vacuoles. Scattered atrophic fibers and syncytial knots are seen. Occasional muscle fibers are completely replaced by storage material which stains in a purple monochromatic color. (H&E)
