Case 6 History ---- The patient is a 56-year-old man with proximal upper and lower limb weakness, neuropathy, elevated CK levels with suspicion of “Inflammatory Myopathy with Vacuoles, Aggregates & Mitochondrial Pathology´ (IM-VAMP).. EMG showed spontaneous activity with mixed myopathic and neurogenic motor units. Serum testing was normal with neurofilament light level of 21. Rule out changes consistent with inclusion body myositis. ---- 6A1A,B-3 H&E stained sections show skeletal muscle with endomysial chronic inflammation, numerous atrophic fibers, foci of endomysial fibrosis, extensive degeneration/ regeneration, extensive irregular clear vacuoles, increase in the number of internalized nuclei, and occasional fibers undergoing myophagocytosis. These findings are indicative of an active inflammatory myopathy. (H&E)
