Sampling of autophagic vacuoles in this case. (electron micrographs) ---- Interpretation: NM Division: Immune stains in both muscles are consistent with “Inflammatory myopathy with vacuoles, aggregates and mitochondrial pathology” (IM-VAMP), a pathologic set of features often seen with clinical inclusion body myositis syndromes. Pathology includes SMI-31 and LC-3 aggregates, diffuse upregulation of MHC Class I by all muscle fibers, and CD 4 and CD 8 cells scattered and clustered in the endomysium, and focally invading a few muscle fibers
