Case 9 History (AANP 1999-2; Contributed by J Bilbao and G Moddel) ---- A 71 year old male presented in May 1996, with a two-year history of increasing difficulty walking, which had started weeks after he was placed on 20 mg of omeprazole and 10 mg of cisapride per day for gastric complaints. Examination revealed 3+15 power of his limbs and no fasciculations or neurogenic signs. Investigations showed a CPK of 170 and EMG which indicated a myopathy. Open muscle biopsy was performed. Diagnosis: Autophagic-vacuolar myopathy induced by omeprazole. ---- 9A1-6 The muscle biopsy shows clear pathology in which vacuolation is accompanied by small basophilic crumbs around vacuoles which are often located around the periphery of the fibers.
