(electron micrographs) ---- Not Shown: Serum neurofilament light chain (NfL) levels of this patient were increased [152(pg/ml): (normal for 41-60 year old controls is <20)]. IgG in this serum binds to Mi-2b antigen by Western blot; such Mi-2b antibodies are associated with dermatomyopathy syndromes including dermatomyositis with muscle fiber necrosis. ---- Comment: Dermatomyositis with abundant inflammation, often perivascular. Inflammatory, histiocytic and lymphocytic, pathological changes include abnormal vessels surrounded by lymphocytes and especially histiocytes. There are atrophic perifascicular muscle fibers, and a few probable necrotic fibers in some fascicles, The muscle fiber and inflammatory changes are common in muscle pathology related to Mi-2b antibody-associated clinical dermatomyositis syndromes.
