Case 3 History ---- The patient is a 21 year old man with significant proximal greater than distal weakness that developed over several months. He also has a rash across his knuckles and abdomen and recent significant weight loss. Clinical differential diagnosis includes an inflammatory myopathy. Operative procedure: Right deltoid muscle biopsy. ---- 3A1-4 Microscopic examination shows striated muscle fibers arranged as perifascicular atrophy. Inflammatory cells are seen in the endomysium and focally perivascular inflammation is present. (H&E)
