Case 5 History ---- The patient is a 59 year old woman with myopathy. Operative procedure and findings: Tracheostomy. Right quadriceps biopsy. ---- 5A1-3 Striated muscle fibers with moderate to marked atrophy, including a pattern of perifascicular atrophy in some areas. There is a marked increase in the number of internal nuclei and there are scattered syncytial knots. The presence of perifascicular atrophy suggests the possibility of dermatomyositis, although no active inflammation was seen in the current biopsy.
