TRI are identified in this small capillary sized vessel. (electron micrographs) ---- Not shown: Ulex shows a mildly reduced number of endomysial capillaries with scattered muscle fibers having no adjacent vessel. MHC Class I is upregulated on the surface of all muscle fibers and commonly in the cytoplasm. CD 4 stains cells in the endomysium and perimysium associated with a few areas of muscle fiber atrophy. LC-3 stains aggregates in muscle fibers, often at the edge of fascicles but in other areas in clusters. ---- Comment: The prominent weight loss in this patient suggests the possibility of an occult neoplasm.
