Case 16 History (Thanks Dr. Chunyu (Hunter) Cai) ---- The patient is a 3 year old boy born at 35 weeks gestation with Apgar scores of 8 and 9 who had difficulty with feeding at birth and developed jaundice. He presented with developmental delay and hypotonia. Biopsy of the left vastus lateralis in August 2010 showed extensive myocyte necrosis and perivascular and intramuscular lymphoplasmacytic inflammation. The patient was treated with steroids. The myositis panel performed in November 2010 was negative. There was clinical suspicion of a mitochondrial disorder. Operative procedure: Muscle biopsy of right quadriceps. ---- 16A1,2 Microscopic examination of the "right quadriceps muscle" biopsy material shows striated muscle fibers with mild fiber size variation. Compared to the patient's left vastus lateralis biopsy performed in August 2010, there is no evidence of inflammation or regenerating/degenerating fibers. In addition, plastic sections show mild increase in the numbers of lipid globules and dark cytosomes.
