There are many abnormally shaped mitochondria with evidence of scattered expanded cristae. (electron micrographs) ---- Not shown: There are no distinctive morphologic abnormalities on Gomori trichrome. COX shows no muscle fibers with reduced staining and scattered muscle fibers may have mildly enlarged mitochondria. There is no mitochondrial proliferation of SDH. There is no storage material on PAS. ---- Interpretation: Non-diagnostic muscle biopsy. These findings represent mild mitochondriopathy but do not reach the level of pathology typical of mitochondrial myopathy. Mitochondrial oxidative enzyme activities are normal. Mildly high citrate synthase levels did suggest mitochondrial proliferation.
