Case 17 History ---- The patient is a 47 year old woman with muscle weakness in the eye along with asymmetric progressive bilateral ptosis and decreased levator function. Clinical impression: Chronic progressive external ophthalmoplegia. Operative procedure: Right deltoid muscle biopsy. ---- 17A1A-C Electron microscopy performed on the muscle biopsy material showed subsarcolemmal clusters of mitochondria. In some, an abnormal circumferential cristae pattern is observed. Many mitochondria also contained rhomboid shaped inclusions. Increased numbers of lipid droplets were seen among mitochondrial aggregates. Overall, there is increased glycogen in the muscle. In some areas there is a slight diminution of the normal amount of myosin filaments. (electron micrographs)
