Many mitochondria also contained rhomboid shaped inclusions and circumferential cristae. Overall, there is increased glycogen in the muscle. In some areas there is a slight diminution of the normal amount of myosin filaments. (electron micrographs) ---- Not shown: Gomori trichrome stain shows scattered muscle fibers with irregular internal architecture, possibly ragged red muscle fibers. SDH shows many muscle fibers with dark staining suggesting mitochondrial proliferation. There are many COX negative muscle fibers but only an occasional muscle fiber with dark COX staining. Sudan shows increased lipid but there is no storage material on PAS. ---- Comment: By ultrastructure this was placed in the mitochondriopathy bin but reached the status of mitochondrial myopathy with the additional clinical and histochemical findings.
