An additional example. (electron micrographs) ---- There are multiple pathologies ongoing in this biopsy. Ultrastructural examination showed no evidence of tubuloreticular inclusions within the cytoplasm of endomysial endothelial cells which can be seen in dermatomyositis. Mitochondria were generally of normal abundance and morphology. However, mitochondrial paracrystalline inclusions were identified in two separate myocytes. Such inclusions can be seen in a range of pathological contexts including mitochondrial disorders. There is no mitochondrial pathology with SDH or COX stains ---- The final diagnosis on this patient was Inflammatory vasculopathic neuropathy with lymphohistiocytic cellular foci, primarily associated with vessels in epineurium, perineurium and endoneurium, marked variability in the degree of axon loss among and within fascicles and subperineurial edema. Other muscle diseases may have substantial secondary mitochondrial changes.
