The pathologic changes in this muscle were similar to the paraspinous but significantly less frequent. (electron micrograph) ---- Comment: These mitochondrial abnormalities have been called mitochondriopathy by us and “mitochondrial ultrastructural cytopathy” by others. The changes are not as compelling as the giant misshapen mitochondria with marked internal alterations such as “parking lots” and intramitochondrial inclusions which are seen in classical mitochondrial encephalomyopathy. The deltoid muscle has similar changes in comparison to paraspinal but to a lesser degree. NM Division reports no clue to the etiology underlying the patient's neck weakness or evidence of a primary myopathy in this muscle. There are non-diagnostic immune stains.
