2A0 Case 2 History
Case 2 History ---- The patient is an 18-year-old man with a history of "respiratory distress." The patient has a history of lung transplant for bronchiolitis obliterans, autosomal dominant polycystic kidney disease, hypogammaglobulinemia, and Asperger syndrome. He recently presented with a respiratory infection with positive nasopharyngeal swab for rhinovirus, complicated by acute respiratory failure of unclear etiology requiring intubation. The patient subsequently was unable to be weaned from ventilator support due to absence of spontaneous movement following withdrawal of paralytic medications. The clinical differential diagnosis included critical illness myopathy and EMG findings were consistent with this diagnosis. Aldolase was elevated and CK was within normal limits. Operative procedure: Left quadriceps muscle biopsy.
