Case 4 History ---- The patient is a 3-year-old boy who has been hospitalized in the ICU in critical condition with liver failure due to Alpers-Huttenlocher syndrome and progressive circulatory failure treated with norepinephrine, epinephrine and dopamine. TPN was stopped because of hyperkalemia. He received insulin and dextrose for this and then a dextrose-containing fluid with sodium but no potassium. White blood cell count reached 50.2K with a hemoglobin of 8 and a platelet count of 105 and his INR remained dramatically elevated at 8. The patient was treated with broad-spectrum antimicrobial therapy but did not survive and was not autopsied. (CANP DSS 2009 Case 7)
