Case 9 History ---- The patient is a 26 year-old-man with a history of congenital heart defect s/p Fontan procedure who was admitted with Fontan-associated liver disease. Ultimately, the patient underwent a combination heart and liver transplant in June 2026 but remained critically ill. There is concern for critical illness myopathy due to steroid Rx and chronic immune suppression. ---- 9A1,2 H&E stained sections show scattered pale cells. Are they necrotic, suffering myosin loss, both or neither? (H&E)
