The patient is a 12 year old non-immunocompromised boy with a one week history of progressive left sided weakness and T2 predominantly white matter signal abnormalities on MRI. CSF studies were negative. Operative procedure: Craniotomy with biopsy. ---- Following this procedure, which resulted in a diagnosis of ADEM, the patient showed rapid clinical improvement along with significant improvement on brain MRI, normal very long chain fatty acids and normal cortisol, all supporting a diagnosis of ADEM.
