Ki67 only highlights inflammatory cells and the total proliferation rate is less than 1%. Not shown: CD138 highlights clusters of perivascular and intraparenchymal plasma cells. There is no evidence of IDH1 (R132H) mutated cells. GMS, PAS and AFB stains were performed and show no organisms. ---- Comment: The collective morphological and immunohistochemical features are consistent with an inflammatory demyelinating process.
